Late diagnosis of posterior urethral valves: a case report and the literature review

Posterior urethral valves (PUV) are the most common cause of lower urinary tract obstruction in boys. They usually suspected antenatally or soon after birth as severe hydronephrosis, oligohydramnios, renal hypodysplasia, and kidney failure. In milder forms PUV, uncharacteristic symptoms occur later childhood, adolescence, even adulthood diagnostic problems. The final diagnosis causative treatment PUV possible due to cystourethroscopy. Nevertheless, a consequence defect, valve bladder syndrome (VBS) occurs requires long-term treatment. authors present case 14-year-old boy with late at age 7. Recurrent pyelonephritis nonspecific voiding dysfunction were first symptoms. Ultrasounds(US) initially normal while videocystometry(VCM) showed 3rd-grade right-sided vesicoureteral reflux (VUR), diverticulum well discrete detrusor overactivity, reduced capacity without any signs obstruction. Bilateral post-inflammatory nephropathy completed clinical picture. urethrocystoscopy surprisingly found incised. Due recurrent infections (UTIs) one year STING procedure was performed which resulted resolution VUR. Despite this, features dysfunction, requiring regular urodynamic evaluation pharmacotherapy, persisted. UTIs boys may suggest should be considered regardless patient's age. Early appropriate management improve prognosis.
